National Conference to End Health Disparities

This event, Faces of Healthy Future, will bring together over 600 health care providers, professionals, researchers, policymakers, community leaders, and program managers from across the nation to dialogue and share critical knowledge, with a focus on best practices, model programs and successful community partnerships to elimiate health disparities.
